Are you an established healthcare provider who has transitioned to offering Telehealth therapy and wondering if you are documenting these sessions appropriately? Are you a newly licensed clinical therapist, who is struggling with documentation in general? Are you looking to brush up on some documentation pointers? Remember the adage: if it’s not written, it didn’t happen? Today, clinical documentation has changed in order to align more with technology standards. This presentation will offer a basic review of the SOAP style of clinical documentation: Subjective, Objective, Assessment, and Plan. This presentation will cover minimal documentation needs for health insurance claims, using the SOAP model. This presentation will also cover other topics related to documentation, including active risk, medical conditions, and documenting contact with clients. There will be ample time for questions, and for case studies.
